Aliquippa woman faces charges in 3-month-old daughter’s cocaine death

An arrest warrant was issued for an Aliquippa woman whose 3-month-old daughter died from cocaine toxicity, according to Aliquippa police and Beaver County detectives.

Rachel Dlubak, 40, was charged Thursday with involuntary manslaughter, reckless endangerment and two counts of child endangerment.

The girl was unresponsive when emergency responders arrived at Dlubak’s home Oct. 30, according to court papers. She was taken to UPMC Children’s Hospital where she was put on a ventilator. The girl was identified as T.H. in court papers. An obituary identified her as Taniyah Harvey.

She died Nov. 4.

Dlubak told investigators she put the baby down for a nap and noticed about 30 minutes later that the girl was not breathing, according to court papers. Dlubak tested positive for cocaine and marijuana and didn’t have any answers for authorities when asked how Taniyah ingested cocaine.

“She had no explanation for how T.H. tested positive for cocaine but reiterated that she was with T.H. that entire morning,” police wrote in the complaint.

Dlubak did not have an attorney listed in online court records.


Police say Murrysville man recorded sexual assault of teens

A Murrysville man is behind bars after Pennsylvania State Police said he video recorded himself sexually assaulting two 16-year-old girls and sending one of them inappropriate videos and photos.

Kevin M. Hullenbaugh, 30, was arrested Thursday on charges of child pornography, unlawful contact, corruption of minors and related offenses.

Troopers said a parent of one of the girls reported the video in which a distinctive tattoo on Hullenbaugh’s arm is visible, according to court papers.

He was being held at the Westmoreland County Prison on $40,000 bail. A July 24 preliminary hearing is set. He did not have an attorney listed in online court records.


Ross police investigate vehicle break-ins

Ross police released surveillance video on social media late Thursday that showed a person dressed in all black pulling on vehicle door handles in the Laurel Gardens neighborhood.

The incident happened around 4:30 a.m. Thursday. Police reported that several residents had items taken from their unlocked vehicles.

They asked residents who have home surveillance systems to check their cameras for additional video that could help investigators identify the culprit.


Police search for suspects in Mt. Lebanon guitar theft

Mt. Lebanon police are looking into a theft of at least four high-value guitars from Empire Music on Washington Road.

Police said three people wearing masks were involved in the Wednesday evening incident.

The individuals didn’t appear to be armed, police said. Authorities are reviewing surveillance video.

Police plan to cite trucker in harassment incident

State police said they plan to file a harassment charge against an 57-year-old trucker from Indianapolis in connection wtih an incident last month in South Huntingdon.

A woman reported to troopers that she was pumping gas at a station on Motordrome Road between 5:30 and 6:30 p.m. June 28 when she noticed the man was watching her.

“He then approached her, hugged her and made small talk,” troopers wrote in a news release.

The trucker touched the woman inappropriately and requested sexual favors in his truck at 9 p.m., according to police. He drove away but authorities said they were able to identify him through his employer and video surveillance. The charge had not been filed.
